Dear former patients of Dr. Kut,

As many of you know, her practice is now closed due to her tragic death. Firstly, I want to thank all of you for the tremendous outpouring of prayers and support that you have extended to her family. Rest assured, she loved all of her patients very deeply.

While the strain on both her family and patients have been enormous. I want you to know that all medical records have been moved to a secure location. They are being converted to electronic format to facilitate transfer to other physicians or for patients who may desire a copy.

The following page is a release form that must be completed for the records to be released. Initially, there may be a delay in receiving records due to the massive number of charts as well as their size. Should your physician require them urgently, please have them contact us directly or make note of this on the release and we will prioritize their transfer. Otherwise, they can be requested later as they will be retained for 10 years.
